    Biography

    Susan Riddick-Grisham, RN, BA, CCM, CLCP, has a private case management and life care planning practice and is the founder of the Care Planner Network, a nonprofit organization. In 2006, she received the Lifetime Achievement Award from the sponsors of the annual International Life Care Planning Symposium.

    Laura M. Deming, MS, RN, PNP-BC, CCM, CLCP, is a practicing pediatric nurse practitioner with a consulting business that includes life care planning services. She received the Case Management Society of America’s Award of Service Excellence in 2004. Laura is also the mother of a pre-school child with special needs.
